As a local to erie, I see a lot of newbies to steelheading, fish for fish in lower clear water, sight fishing, often using a tight line dragging method to their flies as you explained, this results in what is called "Lining" or "Lifting". The fly will generally be on the outside of the mouth. The fish act and fight as if they are fairly hooked but in the long run, that fish did not eat your presentation, the line went through its mouth, creating drag to the line, pulling your fly into its mouth. Just explaining what others are discussing.
