Can anyone please post a pattern/recipe/picture of an October Caddis effective for the Slate Run PA area.

I tie an orange body stimulator on a size 14 200R hook. You can look up a stimulator pattern on the web. I eliminate the palmer hackle, but that is your choice.
 
I assume that by "October Caddis" you mean "Great Autumn Brown Sedge" (the October Caddis is a western fly, but GABS, which we have around here, is often called October Caddis.)

They're big bugs. I just use an orange bodied Elk Hair Caddis in a size 10 or 12 for the adult, or a size 10 orange bodied flymph (partridge hackle) for the pupa.
 
The 200R hook is 3X long. Many (including me) like to tie stimulators with this hook.

Yeah, 3x would make it the length of a 11 hook, which is about right.

Personally, I'd rather have the larger gape, though, of a actual 10 or 12. Goes along with my general philosophy that it's better to tie small on a large hook than to tie large on a small one. YMMV.
